  5. Directors and Officers (names and addresses)
  6. Statutory Agent (name and address)
  7. Identification Number (Form SS-4)
  8. Workmen's Compensation (file for coverage)
  9. Unemployment Compensation (file for coverage)
  10. State Unemployment Compensation
  11. Federal Unemployment
  12. Election under Subchapter S (if going to elect, prepare and file Form 2553)
  13. Accounting Methods (year-end accrual or cash basis, depreciation options, inventory valuation)
  14. Notify Utilities
  15. Banks (select bank or banks and furnish resolution authorizing persons to sign checks and negotiate loans)
  16. Officer's Salaries
  17. Books and Records (chart of accounts, ledger, journals and software)
  18. Insurance (notify all pertinent insurance agents so that insurance policies may be transferred to the corporation if incorporating existing business; if new business, secure necessary insurance, i.e., fire, liability, etc)
  19. Special Licenses and Permits (secure, if necessary)
  20. Accident and Health (formally adopt a plan under 105 ( c) and 106, if desired)
  21. Group Term Life (consider desirability of instituting)
  22. Pension and Profit Sharing Plan
  23. Car Requirements (if employee is furnished company vehicle formally restrict personal use to commuting to work)
  24. Employment Agreement
  25. Redemption or Buy/Sell Agreements
  26. Lease:
    • Personal property
    • Real estate
  27. Corporate Records (stock certificates, stock register, minutes, corporate seal)
  28. Stock Issue (issue shares of stock bearing necessary signatures and with corporate seal affixed; complete stock register; obtain receipts or have stubs initialed)
